Delivery of Documents

Shareholders,

Without your consent, it is mandatory to mail each shareholder a paper copy of Fund materials, including proxy-related materials, semi-annual and annual reports, financial statements and the Management Report of Fund Performance.  

You may choose to receive these materials electronically instead.  

By completing the form below, you will receive documents by email and notifications when reports are available online—reducing paper mail and ensuring timely access to your investment information.

I have read and understand this “Consent to Electronic Delivery of Documents” and consent to the electronic delivery of the documents (and/or types of documents) listed below that SaskWorks Venture Fund Inc. elects to deliver to me electronically, all in accordance with my instructions below.

  1. The documents that are covered by this consent to electronic delivery are the reports (annual report and semi-annual report), proxy-related materials, financial statements and Fund updates.
  2. The reports to be delivered electronically shall be posted to the website www.saskworks.ca. A notification via electronic mail (“e-mail”) will be sent to the e-mail address that I provide to SaskWorks Venture Fund Inc. advising me that the reports have been posted to the website and provided me a link to the website.
  3. I understand that I will require access to the internet in order to retrieve the documents that will be electronically delivered and I confirm that I have such access.
  4. I further acknowledge the documents will be posted to the website in Portable Document Format (PDF), and the Adobe Acrobat Reader® software is available free of charge from Adobe’s website www.adobe.com.
  5. I acknowledge that I may request and receive at no charge, a paper copy of any documents delivered electronically by contacting SaskWorks Venture Fund Inc. at:

  6. I understand and agree that the reports will be made available on the website for a period of no less than 6 months, and after that period they remain available at www.sedar.com. I also agree at any time without giving me advance notice SaskWorks may elect, at its sole discretion, not to send documents electronically, in which case a paper copy of the document will be delivered according to applicable legal requirements.
  7. I understand that I will be provided with a paper copy of any documents delivered electronically if electronic delivery fails.
  8. I understand that any personal information I provide to SaskWorks on this form, including email addresses and other information, will be used solely for the purposes described in this form.
  9. I understand that my consent may be revoked or changed, including any change in the electronic mail address I have provided, at any time by notifying SaskWorks of such revised or revoked consent by telephone, regular mail, or electronic mail.
  10. I understand and agree that I am in no way obligated to consent to electronic delivery.

I am a shareholder of SaskWorks. I have read and understand the terms of this “Consent to the Electronic Delivery of Documents” form and, on these terms, I consent to the electronic delivery of the documents I am entitled to receive. I understand and agree that this consent will remain in effect for the entire time I am a shareholder or until I revise or revoke my consent to electronic delivery.
